If you are an existing or new Conte Dental patient, you will please need to fill out your patient forms 1-7 days prior to your appointment. This must be filled out before you arrive at our Bonita Springs office. Providing your medical and health history allows us to attend to your medical needs more quickly and more efficiently.
It's Easy!
*Existing Patients (patients of record)**
Step #1: on contedental.com homepage, click on the Existing Patient Forms link.
Step #2: sign up for the Patient Portal. Once you sign up, you will receive an email verification code to ensure your identity. Follow the instruction to insert the verification code. Once you insert the code, sign in to the patient portal to view online forms.
Step #3: login to the Patient Portal. Online Forms will appear in the left side column when using a computer. Click on the tab. Your information will be filled in. Please check the information to make sure it is correct.
Our office will automatically be notified when your forms have been completed.
**New Patients **
On contedental.com homepage, go to Patient Forms tab and click on the link for New Patient Forms.
New patients will need to complete two questionnaires:
1. Patient Registration
2. Patient Medical History
Our office will automatically be notified when your forms have been completed.
Once you have completed your forms, click the contedental.com website link to the Patient Portal. Here you will create a unique username and password that you will use to login and view your information.
Patient Portal
Every patient will have their own unique Patient Portal. Your Patient Portal features:
- your patient information
- view upcoming appointments
- update account information, health history & pharmacy
- billing & treatment statement (coming in 2021)
- HIPPA secure messaging to our office
